Vital Records provide accurate data concerning life events. The state of Georgia maintains vital records from 1919 to the present.
Clayton County Public Health is responsible for filing, preserving, protecting, and issuing certified copies of all Georgia births.
Please be advised: While we are now able to issue death certificates for events that occur within the state of Georgia, we cannot guarantee immediate issuance of the certificate if the event occurred in another county. Most of the time,services are immediately available. Occassionally it may take up to 3-5 business days to receive your certificate.
How do I request a vital record?
When requesting a birth or death certificate,you may pay by Cash, Credit/Debit Card or Money Order.
Please provide the following to receive a certified copy of a birth or death certificate.
Search fees(includes one certificated copy,if the record is found on file): $25.00
Additional certifications of same record ordered at the same time: $5.00
*Georgia law requires the $25.00 fee be received before a certified copy of a birth or death certificate can be issued.
Payment of the fee is due at the time of service and search fees are Non-refundable.
The Vital Records Department is open from 8:00am to 4:00pm.
