Child Immunizations

One very important step in ensuring the health of your child is making sure he or she receives their immunizations on time.   There are 14 diseases that can be prevented with routine childhood vaccination.  CCBOH offers vaccines to children 0-19 years of age.  CCBOH is also a VFC (Vaccines for Children) program provider.


Immunizations are available by appointment or walk-in (based on the total number of clients being serviced) Monday-Friday 8:00am-4:00pm.

Georgia's immunization requirements for students entering or transferring into the eleventh grade have been revised to align with the current Recomendations of the Advisory Committee on Immunization Practices (ACIP), the American Academy of Pediatrics (AAP), and the American Academy of Family Physicians (AAFP).

Effective July 1, 2020, all students who are new entrants or transfers into a Georgia school in the eleventh grade, will require proof of a booster dose of the meningococcal conjugate vaccine, unless their first dose was received on or after their sixteeth birthday.

  • Copy of Immunization records
  • Insurance/Medicaid Information
  • Child must be present at time of service


Children 18 years and younger MUST be accompanied by a parent or legal guardian/proxy to receive services.

Cash, credit or debit, Visa, Mastercard, Discover and Flexible Spending Account Cards.

Georgia Medicaid, PeachState, WellCare, Amerigroup, BlueCross BlueShield, Aetna/Coventry, United Healthcare, Cigna and Medicare.
